Some other things you should be aware of:
The Rule ONLY applies to flights that are canceled at the fault of the airline. Air traffic control, weather, or acts of God or the government do NOT invite the application of this rule.
Tickets booked through consolidators (especially internet ones) are usually NOT included.
Free tickets, tickets with mileage or vouchers, are NOT included.

The US Airways change on unaccompanied minors was probably simply adopting the America West rule in the merger (they got pretty tight after losing a few kids…)

More important than waving a document in a gate agent’s face that may or may not get you anywhere is having a clear, actionable plan. In the event of major problems with your flight, that means knowing alternative flight options and routings. It’s a great move to always carry a copy of that day’s schedule between your origin and destination. I’ve used this many times:

(appears to be down at the moment… hopefully it’s a temporary thing)
It’s also great for knowing if there’s an earlier option for standby, another routing on the same airline (Minne. closed by snow? Let’s try Detroit!), or when to expect bags that somehow end up on the flight after yours.

And even more importantly, download the contract that was in effect when you bought your ticket. I can tell you from experience that the airlines change their contracts whenever they get caught in an expensive got cha. However, the contract was in effect when you bought the ticket controls the substantive elements of your agreement with the airline. Sound like a pain, yep, it is. Just try keeping track if you fly all the time.
